Terms & Conditions

Your mental health matters to us. These Terms & Conditions explain what you can expect from our services and help ensure a safe, respectful, and supportive experience throughout your care. 

New Patient Information

Before Your First Appointment

To ensure we have everything needed for your visit, all required intake forms and consent documents must be completed at least 24 hours before your scheduled appointment.

This includes:

  • Patient registration forms
  • Medical and psychiatric history
  • Insurance information (if applicable)
  • Payment information
  • Required consent forms

Failure to complete the required paperwork before your appointment may result in rescheduling.

Initial Psychiatric Evaluation

Your first visit is a comprehensive psychiatric evaluation. During this appointment, your provider will discuss your concerns, review your medical and mental health history, and develop an individualized treatment plan.


Treatment recommendations may include:

  • Medication management
  • Therapy referrals
  • Lifestyle recommendations
  • Additional evaluations or testing
  • Referral to another specialist when appropriate


Please note that completing an initial evaluation does not automatically guarantee ongoing treatment or the prescription of medication. Every treatment decision is based on clinical judgment and your individual healthcare needs.

Communication Policy

We strive to provide timely communication while protecting your privacy.


Phone Calls


Our office reviews voicemail messages during regular business hours.

  • Non-urgent calls are generally returned within two business days.
  • Messages received after business hours, weekends, or holidays will be addressed on the next business day.

If you are experiencing a medical or psychiatric emergency, please call 911 or visit your nearest emergency department immediately.

Text Messaging

With your permission, we may send text messages regarding:

  • Appointment reminders
  • Scheduling updates
  • Administrative notifications

Please do not send confidential medical information or urgent clinical concerns through text messaging, as text messages are not considered a secure form of communication.

You may opt out of text messages at any time by replying STOP.

Patient Portal

Our secure patient portal is the preferred method for communicating about your care.

You may use the portal to:

  • Request medication refills
  • Ask non-urgent clinical questions
  • Update personal information
  • Complete required forms
  • View documents when available

Portal messages are typically answered within two business days.

Email Communication

Email should only be used for general administrative questions such as scheduling or billing.

For your privacy and security, please do not send personal medical information or urgent concerns by email.

Appointment Policy

Your appointment time is reserved specifically for you. If you need to cancel or reschedule, we kindly ask that you provide at least 24 hours' notice. Patients arriving more than 10 minutes late may need to reschedule their appointment depending on the provider's schedule. Repeated missed appointments or excessive late cancellations may result in dismissal from the practice so that appointment times remain available for patients actively engaged in treatment.

Medication Management

Medication decisions are based on your individual symptoms, diagnosis, medical history, and treatment goals.

To ensure safe and effective treatment:

  • Take medications only as prescribed.
  • Notify your provider of any side effects or concerns.
  • Attend recommended follow-up appointments.
  • Do not stop medications without consulting your provider unless instructed to do so.

Prescription Refill Policy

Please request medication refills before your current prescription runs out.

Most refill requests are processed within two business days.

Certain medications may require a follow-up appointment before additional refills can be authorized.

Early refill requests, replacement of lost or stolen medications, or refill requests made outside of established treatment guidelines may not be approved.

Controlled Substance Policy

Certain medications require additional monitoring to ensure safe, effective, and responsible treatment.

Examples may include medications prescribed for:

  • ADHD
  • Anxiety
  • Sleep disorders
  • Other conditions requiring controlled medications

Depending on your treatment plan, your provider may require:

  • Monthly follow-up appointments
  • Urine drug screening
  • Prescription monitoring reviews
  • Psychological testing when clinically appropriate

Please note:

  • Early refills are not provided.
  • Lost or stolen medications cannot be replaced.
  • Controlled substance prescriptions may require periodic in-person visits, even if most care is provided through telehealth.

Failure to follow treatment recommendations or medication agreements may result in changes to your treatment plan or discontinuation of controlled substance prescribing.

Billing & Insurance

We are committed to making the billing process as transparent as possible.

Patients are responsible for understanding their insurance benefits, including:

  • Copayments
  • Coinsurance
  • Deductibles
  • Non-covered services

While we are happy to assist with insurance verification and prior authorization requests, insurance approval and payment decisions are determined by your insurance carrier.

Payment for patient responsibility amounts is due according to your insurance plan or self-pay agreement.

Patients with outstanding balances may be asked to establish a payment arrangement before scheduling future appointments

Self-Pay Patients

For patients without insurance coverage or those choosing to self-pay, payment is due before services are provided unless otherwise arranged. Current self-pay rates are available by contacting our office.

Medical Records

Your medical record is an important part of your healthcare.

You may request copies of your records as permitted by applicable laws.

A signed authorization may be required before records can be released to another individual or healthcare provider.

Processing times may vary depending on the nature of the request.

Privacy & Confidentiality

Protecting your personal health information is one of our highest priorities. Lumen Psychiatry complies with all applicable HIPAA privacy and security regulations.

Your information will only be shared when:

  • You provide written authorization.
  • Disclosure is required or permitted by law.
  • There is concern for your safety or the safety of others.
  • Reporting of abuse or neglect is legally required.
  • Required for healthcare operations, payment, or insurance purposes.

We are committed to maintaining your privacy while ensuring you receive safe, coordinated care.

Non-Voluntary Discharge Policy

 Lumen reserves the right to discharge patients through written notice for reasons including:

  • Failure to comply with treatment recommendations 
  • Verbal abuse toward staff 
  • Physical abuse 
  • Threatening behavior 
  • Violent or perceived violent conduct 
  • Suspicious or unsafe behavior 

In situations involving safety concerns, law enforcement may be contacted.
